Where can I find the names of the continents?

Ask someone, that's the simplest way.

The one that Anglyn, Aquitania, Josepania, Bretaigne, Sorlandeten-Serenierre, and the memorial territories is 'Boreas'.
The big southern one with Hajr, Frescania, and Carentania on it [as well as others] is Himyar.
The europe looking on e with Wiese, Montelimar, Engellex, Franken, etc, has no official IC name but is called by some 'Gallo-Germania'.
The island continents off of that continent's area are 'Scania'.
The eastern portion of the 'gallo-germania' continents are first the midlands, and then on the far end:
They're Toyou/etc.
There is currently no official name for the continent that Askwesasne is on, and Cangintgy area/the ones marked 'O' are 'Oceania'.

I don't think that list is too important, and, by the way, I was attempting light humour - hence the wink.

Preuti-Borussia, is what my nation calls Gallia-Germania, like Saasmiskavia is Scania.
